Kohala [1]

The main island of Hawai'i is composed of 5 volcanoes (Kohala, Mauna Kea, Hualalai, Mauna Loa, and Kilauea), of which Kohala was the first to break the ocean's surface. [2]

Technically this main island isn't the first island created nor is this volcano the progenitor of the entire Hawaiian island chain (in the variant question 'which volcano was responsible for the formation of current day Hawaii') - the Hawaiian hotspot is responsible (in theory) for the formation of the various volcanoes which subsequently created the chains and the island of Hawai'i itself.
